PLEASE READ THIS AGREEMENT CAREFULLY
RESERVATIONS
FOR ANY HOTEL RESERVATION MADE BY YOU ON THIS SITE, UPON CONFIRMATION OF YOUR RESERVATION, YOUR CREDIT CARD WILL ONLY BE USED TO SECURE THE RESERVATION. CHARGES WILL BE MADE UPON CHECK-IN FOR FULL PAYMENT/LENGTH OF RESERVED STAY OR UPON FAILURE TO CANCEL AS OUTLINED BELOW. IN ORDER TO PROCESS YOUR RESERVATION REQUEST, YOU ARE REQUIRED TO PROVIDE DETAILS OF YOUR CREDIT CARD. SUBMISSION OF YOUR REQUEST SHALL BE CONSIDERED TO BE FINAL AUTHORIZATION BY YOU.
Upon your check-in date, you shall present yourself personally along with the credit card on your reservation, and other necessary identification documents for the purposes of confirming your identity and reservation. Check-in will not be permitted if the name on identification document and Credit Card DOES NOT match with the name on your reservation.
You may request to check in up to 24 hours in advance of the patient’s first scheduled admission date. You may request to check out no later than 24 hours after the patient discharge date. Appointment and surgery dates will be confirmed prior to your reservation being confirmed. If admission is changed or cancelled, this may result in your reservation being changed or cancelled.
ELIGIBILITY OF STAY
You must be a family member or healthcare advocate for a patient being admitted for care. Upon request, proof of relationship to patient must be provided when making room reservations.
LENGTH OF STAY
Stays of one (1) to four (4) nights may only be booked 48 business hours in advance. Stays of 5+ nights may be booked up to 1 month in advance. If your stay will require 5+ nights and it is less than 30 days of the date of admission, please call our office at 229-259-4410 to determine availability.
MODIFICATIONS/CHANGES for SHORTENING OF STAY
You are liable for any additional charges or expenses imposed by SGMC Family Pavilion in respect to such modifications/changes for shortening of stay and/or full cancellation based on our current Cancellation Policy.
OCCUPANCY
We offer up to two (2) rooms/reservations per patient based on availability. Children under the age of twelve (12) years of age are not permitted to stay in guest rooms. All minors must be accompanied by an adult at all times. Rooms accommodate up to two (2) people (including minors).
FLOOR VISITATION
In an effort to maintain the integrity of offering privacy to our guests, only those who are listed on the room reservation are permitted to be on the Family Pavilion floor. All other visitors/visitations should be met in one of our main common areas of the hospital such as, main lobbies, cafeteria, cafe, etc.
FOOD SERVICES
Under no circumstances will guest trays (food service) be delivered by hospital personnel to the guest rooms. Under no circumstances will food delivery from outside of the facility be permitted to the Family Pavilion floor. Should you need any food or beverage SGMC has two dining options on campus. Our main cafeteria and All Spice Café are located on the main level of the hospital. Vending is also located on the first floor.
Self-serve, complimentary coffee is available to our guests in the Common Area’s Kitchen/Pantry. We also have the added convenience of a refrigerator, ice machine, and microwave. We do ask that you assist with maintaining the cleanliness of these amenities as this is a shared service among all guests staying with us. Refrigerators will be emptied on Friday of each week; all items left in the refrigerator on Friday’s will be thrown away. This is to ensure we are meeting our minimum requirement for our Environment of Care policy.
PERSONAL BELONGINGS
Guest rooms are not furnished with personal safes therefore it is encouraged that items of value remain on the guest and are not stored in the room. SGMC is not responsible for personal belongings.
CONDUCT
Please be sensitive and considerate of other families and patients by keeping noise levels down. Smoking, alcoholic beverages, controlled substances, and any controversial activity is prohibited at SGMC. Any suspicion or reports of such activity will give cause to consider termination of the guest reservation. Cancellation is at the discretion of SGMC.
SAFETY/SECURITY/PERSONAL INJURY
SGMC Security reserves the right to access guest rooms under conditions of emergency or reports of extreme concern in regards to our Standards of Stay. SGMC will have the presence of security. In the event you need immediate assistance please dial 4030 from the common area phone or 229-259-4030 from your personal phone. SGMC is not responsible for personal injuries sustained by occupants of our guest rooms.
ROOM CONDITION
Fire Code mandates the layout of our guest rooms. Furniture is not to be removed, replaced, or rearranged. Guests will be responsible for all property damage. Rooms are inspected prior to check-in and at time of check-out to ensure they have been properly maintained.
ALTERATION
We reserve the right to change, amend or alter prices or any conditions stated herein from time to time or without prior notice.
ROOM ACCESS
SGMC's Facility team reserves the right to make entry in the rooms when emergency repairs are needed, reports of concerning activity, and/or security related issues.
Payment Policy
Payment shall be made by major Credit Cards that are accepted by our payment services provider. Payments are therefore subject to the approval of such financial institution/payment services provider and SGMC shall NOT be liable in any way if such financial institution/payment services provider refuses to accept or honor the Credit Card for any reasons. Furthermore, SGMC makes no responsibility to any surcharges made by your Credit Card issuer and/or their partner Banks. Please check with your Credit Card issuer if you have any questions regarding this matter.
SGMC will impose charges for room reservation(s) in full, upon check-in and any incidentals that occur during your stay. Other expenses incurred by you are payable directly to SGMC. For instance, you are required to pay directly to SGMC for any extras such as food and beverages consumed, other charges.
Cancellation Policy
Cancellations of reservations must occur 48 hours prior to your first date of stay. Cancellations after this cut-off will incur charges for the full length of the reservation.
